Summary
The position is located at the Greater Los Angeles Healthcare System West Los Angeles Campus within the Community Engagement and Reintegration Service (CERS). The incumbent is a Licensed Social Worker at the advanced practice level assigned to the Health Care for Homeless Veterans (HCHV) Coordinated Entry (CE) Program. The CE Social Worker is responsible for implementing the VA One Team integration initiative into the Coordinated Entry System.
Duties
- VA Careers - Social Work: major duties and responsibilities include but are not limited to: Enables VA homeless programs to fully collaborate with the community - including the Continuum of Care (CoC) and community partners - in the coordinated entry efforts in that community
- Ensures continuity of care for homeless Veterans between VHA and community-based programs - as the Veteran transitions from homelessness into programs such as HCHV Contract Residential Housing (CRS) - Residential Rehabilitation and Treatment Programs (RRTPs) - and Grant Per Diem Programs (GPD) as well as directly to permanent housing through the Housing and Urban Development- Veterans Affairs Supportive Housing (HUD-VASH) Program - the Supportive Services for Veteran Families (SSVF) - or other forms of housing in the community
- Provides (SSVF) - or other forms of housing in the community
- This continuity of care also includes coordination with the local Suicide Prevention Coordinator(s) (SPC) to enhance suicide prevention amongst homeless Veterans encountered through community-based programs
- Participates in the local plans to end Veteran homelessness as well as conducting and leading the coordination of outreach services to homeless Veterans
- S/he must be able to develop appropriate professional relationships with community stakeholders and is responsible for communicating potential CoC policy or protocol changes to local VA leadership where such changes would enhance the VA's ability to coordinate housing and services for homeless Veterans with the CoC or other key partners
- Facilitates VA involvement in the CoC's Coordinated Entry System (CES) and will be responsible for helping VA meet federal guidelines on CES - as described by regulations and guidance published by HUD
- Coordinate their efforts with local planning and support currently provided by VA - HUD - and USICH technical assistance initiatives designed to support local planning and the creation of a CES
- This involvement requires that the incumbent is both aware and involved in current CoC plans to meet these federal requirements
- Acts as the liaison - participating in By Name List (BNL) case conferencing and other pertinent community meetings - as needed
- Provides case management services to a special patient population that demonstrates highly complex psychosocial - health or mental health problems
- The incumbent provides crisis intervention consultation with contractors as needed
- Provides coordination and providing information and referrals to VA programs to assist contractors in their provision of case management services
- Establishes and maintains effective therapeutic relationships with Veterans and their families when appropriate
- Independently with patients and families who are experiencing a variety of psychiatric - medical - and social problems - utilizing individual - group and family counseling and therapy skills
- Independently provides consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans
- Provides working knowledge and skill in the use of word processing - data management and other computer systems - especially those programs in use by the VA
- The social worker will be able to meet the needs of customers while supporting VA missions
- Consistently communicates and treats customers (Veterans - their representatives - visitors - and all VA staff) in a courteous
- tactful - and respectful manner
- Documents in the patient's medical record in accordance with medical center policies and the requirements of The Joint Commission (TJC) and other regulatory bodies
- CPRS notes are timely and accurate
- Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the Coordinated Entry Specialist for CERS
- Reviews VA data systems such as HOMES and CPRS to improve performance and delivery of services to homeless Veterans and ensuring that this information is shared (in a manner consistent with VA information sharing directives) so that community Master or By-Name Lists are up-to-date and complete
- Works as a field instructor with MSW student interns or clinical supervisor for unlicensed VA staff - when requested
