We are looking for a Remote Client Care Coordinator to assist clients in accessing services and navigating available resources. This position will work closely with clinical and case management teams to follow up on appointments, referrals, and service plans.
Conduct follow-ups with clients regarding appointments and service engagement
Provide reminders, check-ins, and support to improve care plan participation
Update records with service outcomes and contact attempts
Help track completion of assessments, referrals, and client paperwork
Collaborate with clinicians and support teams to improve continuity of care
Maintain confidentiality and handle sensitive information appropriately
2+ years of experience in client support, case coordination, or social services
Clear and compassionate communication skills
Strong organizational skills and experience using client databases
Knowledge of social service or behavioral health systems preferred
Ability to manage tasks independently in a remote setting
